Instructions:

1. All fields followed by a blue asterisk (*) in the form below must be filled in completely.
2. If you a California retailer/wholesaler, please complete and fax the 'California Resale Certificate' to us at (925) 803-9857.
3. When finished, press "Get Samples Now" to get your samples mailed to you. Once we receive your Sample Pack Request Form, you will be invoiced via email for the sample pack selected plus US$14.95 shipping and handling if delivery is in the United States.  International sample pack shipments are sent via FedEx International Economy or Global USPS Priority Mail; rates vary by location. International shipping/handling rates will be listed on the invoice emailed to you prior to shipment.  Payment is made securely by email via the PayPal eMail Payment Service.  Styles and colors vary per sample pack. No two sample packs are alike. A sample pack includes eight (8) pieces of pet jewelry.
